    When visiting a Dr for any reason, i.e. cold, flu, stomach pains, etc. there should be a place that the PT can enter information as to why they went to the Dr. and able to enter the DX and Rx given plus any other pertinent information. A note section could be added in the Day, week, month. Example: Today- note- 5-1-18, went to Dr due to severe sore throat. Dr Dx with toncilities, recommends, gargling with xXxX, Rx antibiotics for 7 days. F/U in 1 week.
