Like many patients, I have generalized pain, with some areas worse than others or some not a problem at a given time. It would be very useful to add a note to the symptom tracking entries that could allow patients to specify areas of the body affected, any specific injuries for those of us who have conditions that frequently cause them (brittle bone, Ehlers-Danlos, etc.), and the quality of the pain (burning, aching, pins and needles, throbbing, etc.) Such notes could be helpful for other conditions as well, such as recording foods that were eaten before entering abdominal pain or acid reflux symptoms or weather conditions associated with lightheadedness. I'm sure there are many more applications I'm not thinking of!

For example, every doctor ask you about date when simptomes or the date when you start with that type of medication. It is very useful to keep log of important changes in your therapy. Thanks

When visiting a Dr for any reason, i.e. cold, flu, stomach pains, etc. there should be a place that the PT can enter information as to why they went to the Dr. and able to enter the DX and Rx given plus any other pertinent information. A note section could be added in the Day, week, month. Example: Today- note- 5-1-18, went to Dr due to severe sore throat. Dr Dx with toncilities, recommends, gargling with xXxX, Rx antibiotics for 7 days. F/U in 1 week.

Very useful for tracking unusual event

i am on a 4 year old therapy with humira injections that i have to make every 15 days. the problem is i always forget which side i made my last injection on. it would be really helpful if i can mark somehow which side i have me the injection in order to alternate from the left side of my tummy to my righ side. thank you!!

A brief comment could help clarify a change of symptoms in the report for the doctor. For example bowel symptoms may be related to having eaten icecream the day before. Or fatigue may be related to having a unusually busy day.

the meds are mood altering, the affliction has auras, I'd like to note what days these things happen to track a pattern. basically a diary.

It would be very helpful to add notes to symptoms. Right now it's to general to be particularly helpful for complicated chronic conditions like MS.
